COMPARATIVE STUDY BETWEEN THE EFFECT OF VOLUME-CONTROLLED VENTILATION AND PRESSURE CONTROLLED VENTILATION VOLUME GUARANTEED ON GAS EXCHANGE AND RESPIRATORY DYNAMICS DURING ONE-LUNG VENTILATION

Background: Impairment of gas exchange is a common problem during one lung ventilation (OLV) with hazardous effect on morbidity and mortality. The current study compares the volume-controlled (VCV) versus pressure-controlled volume guaranteed (PCV-VG) OLV.Settings Design: This was prospective randomized study.Methods: carried out 30 adult patients who were randomly assigned to two groups. Group VCV: VCV performed throughout operation. PCV-VG: PCV-VG Haemodynamic parameters ((HR), MAP), (CVP)), blood analysis, PaO2/FiO2 recorded: before induction anaesthesia (T0); TLV (T1); ½ an hour after OLV (T2); 1 h (T3); 20 minutes resuming (T4); 6, 12, 24 hours surgery (T5, T7). (Ppeak), (Pmean) (Cdyn) recorded (T1, 2, 3, T4) time intervals. TNFα level measured in venous Samples following times: (1) (2) Directly (3) 5th postoperative day (T2). followed for 5 days recording PPCs.Results: leads significant decrease Ppeak Pmean increase Cdyn (P ≤ 0.05). However, it had no intra- oxygenation values.Conclusion: superior as regard respiratory dynamics (OLV), while does not differ items exchange, inflammatory response PPCs.Abbreviations: OLV: ventilation; TLV: guaranteed; PaO2: arterial oxygen tension; FiO2: fraction inspired oxygen; Ppeak: Peak inspiratory pressure; Pmean: mean Cdyn: dynamic compliance TNFα: Tumour necrosis factor α; PPCs: pulmonary complications.
